Achieving Occupational Therapist Licensure

Foreign educated occupational therapists wishing to immigrate to the United States must pass four standardized tests required by US immigration. These are the:

- Certificate of Occupational Therapists Registered Examination (OTR)
- Test of English as a Foreign Language (TOEFL)
- Test of Written English (TWE)
- Test of Spoken English (TSE)


Please note that the OTR and English Language exams are required before a non-immigrant visa application can be submitted.

Instructions on Applying to write the OTR


To receive OTR Certification from the National Board for Certification in Occupational Therapy (NBCOT) is a three step process for all foreign educated occupational therapists.

Step One – Pre-Screening Program
Download and complete the Pre-screening Packet for International Applicants from the NBCOT web site at www.nbcot.org or click here.

Please complete and follow the instructions as outlined in the Pre-screening Packet Instructions. Incomplete forms will cause a delay in processing. Once the NBCOT has received payment of $400.00 and all required documents, it will take approximately 6-8 weeks for review.


Please note that your pre-screening application will not be approved until you have passed all of the English Examinations.

Step Two – English Language Examinations

Apply and write both the TOEFL/TWE and TSE English Language Examinations. Please remember to request that your TOEFL/TWE and TSE scores be forwarded directly to NBCOT from ETS. The NBCOT institution code number is 9953.
The passing scores for the English Language Examinations are:
- TOEFL iBT - Overall 89 and Speaking 26

- TOEFL - 220
- TWE - 4.5
- TSE - 50


Please note that your English results must be received prior to you receiving eligibility to write the OTR Exam.

Step Three – OTR Examination


Once your Pre-Screening Application has been approved the NBCOT will provide you with the International Candidate Handbook

Please read the International candidate Handbook and complete the NBCOT Examination Application form. Once you have completed your Application form please send the application form and payment to NBCOT Application Process Address as indicated in the Handbook on page six. The OTR Examination Application fee is $520.00US (includes adminstration fee) and test results are sent three weeks after the examination.

Once the NBCOT has received your OTR Examination Application forms and payment they will review the application and provide you with your Authorization to Test (ATT). Please note that you will be able to check the status of your application on line. Your ATT will be valid for 180 days. Within this time period you will be required to write the OTR examination.

To schedule your exact date you will need to contact the Prometric Testing Center and inform them which testing center you would like to write the OTR Exam at.   Please note that NBCOT has opened three international testing centers.  These testing centers are in the Philippines, Singapore and in the United Kingdom.   

The scheduled spring dates for the OTR exam at the International Testing Centers are:

Saturday, July 1, 2006 –  Manila, Philippines

Saturday, October 28, 2006 – St. Francis Methodist School, Singapore


OTR Examination Details

The Occupational Therapists Registered Examination (OTR) is a four hour exam consisting of 170 four-option multiple choice items. A passing score of 450 is required in order to receive certification.

Total Licensure Fees

Please find a break down of the total licensure fees for occupational therapists wanting to go to the US.
TOEFL/TWE = $140.00
TSE = $125.00
Pre-screening = $400.00
OTR Exam = $520.00 - included administration fee
Licensure = $250.00 *
